Overview
Responsibilities:
- Work with a dynamic team of professionals including nurses and social workers in our foster care program for children with special medical needs.
- Provide intensive casework services and fieldwork for children and their birth and foster families with the goal of providing permanency as quickly as possible.
- With medical staff, team-train foster parents in specialized child care skills.
- Conduct intensive outreach to connect children and families with agency and community treatment resources.
- Maintain written case records.
- Other duties as necessary.
- Training provided.
- MFC Social worker will work in Partnership with RN assigned to cases. They will work closely in collaboration on cases. Including conducting home visits together whenever possible.
Qualifications:
- Bachelor’s Degree in Social Work or related field required, Master’s degree preferred.
- Foster care experience preferred;
- Excellent verbal, writing and organizational skills;
- Flexibility, and ability to work under pressure and meet deadlines;
- Computer literacy;
- Bilingual (Spanish/English) preferred.
