SENIOR CASE MANAGER

Description:

The Senior Case Manager’s responsibilities include direct care/ case management responsibility for up to ten (10) clients, plus clinical supervision of two case managers. The Senior Case Manager (SCM)’s case management responsibilities include enrollment in and coordination of public benefits, linkages to health, mental health, social services, educational/ vocational programs and other needed services, counseling, and assistance with daily living skills. The SCM will also be responsible for twice annual case record reviews, and will assume the duties of the Clinical Coordinator and/or Program Director in their absence.

The Senior Case Manager will report directly to the Program Director and in the event of his/ her absence, to the Clinical Coordinator and/or Quality Assurance Director.

Qualifications/ Requirements:

Bachelor’s degree, plus 2 (two) years’ work experience in social services, with one year’s supervisory experience preferred

Candidates must demonstrate good verbal and written communication skills, and must have good computer skills 

Schedule: Office hours are Monday – Friday; 9:00 am to 5:00 pm.

Physical Requirements/ Limitations

Candidates must be able to access areas above first floor of facility and must be able to demonstrate proper cleaning and apartment maintenance methods to residents in addressing their activities of daily living.

Salary: Annualized at $56,336 plus benefits
