Intro/Program Description: LSNC is a multigenerational program of Goddard Riverside that serve educational, recreational, cultural needs and aspirations of Lincoln Square community with a focus on residents of Amsterdam Houses and Amsterdam Addition. West 65th Older Adults Center is funded by Department for the Aging. More than 300 active members are offered daily nutritious hot lunch as well as a variety of wellness, educational, art programs and social services. Senior services provide an opportunity for older adults (age 60+) to lead more vibrant and meaningful lives through comprehensive programming and social support including case management, case assistance and health promotion.

 

Purpose of Position: The Clinical Manager for Older Adult Programs provides assessment, services coordination and counseling to social services staff involved in client care. Supervises Case Managers in daily operations to assure that social services and programming are effectively delivered. Provides direct service to clients as per social worker responsibilities. Specific services include: assistance with applications and recertification for benefits including Medicaid and Medicare, referrals and connections to services such as post-discharge homecare, legal services and medical care; advocacy with public and provide systems; collaboration with the on-site nurse; home visits to ensure clients are safe and well.

 

Schedule:  Monday through Friday – 8:00am to 4:00pm, Weekends: Cooling Center Administration if required (occasionally, summer only, rotating schedule);On-site and remotely as scheduled 

  

 

Goddard Riverside and Isaacs Center follow the CDC and NYS recommendations to prevent the spread of COVID-19, and are now requiring all new hires to be vaccinated against COVID-19 unless they have a qualified exemption. 

Role, Responsibilities and Essential Duties

1. Client services: 

  • Carry a reduced active caseload, and/or provide direct assistance with complex cases 
  • Assist and advocate for seniors on a wide range of issues including entitlements, health and mental health care, food assistance and housing 
  • Perform annual assessments and make home visits to homebound seniors 
  • Complete thorough mental health assessments and provide supportive counseling and referrals as needed 
  • Assess clients for cognitive changes and provide intervention as needed 
  • Assess for potential elder abuse and neglect and work with Adult Protective Services 
  • Work with onsite nurse to coordinate medical care and health management 

 

2. Aging Services team member: 

  • Attend staff meetings, training programs and case conferences 
  • May be asked to coordinate and participate in activities
  • Plan and implement groups in the senior center or remotely as necessary 
  • Assist with outreach and engagement 
  • Provides coverage for social worker if needed 

3. Clinical Supervision / Administration Duties: 

  • Collaborate with Older Adults Program Director /NORC Director for efficient and effective site operation and coordination of services. 
  • Participate in Aging Services program assessments and improvement plans 
  • Assist in development and revision of agency policies and procedures 
  • Assist in daily oversight of senior center and respond to emergencies 
  • Provide clinical support and guidance to Aging Services social work staff, and interns 
  • Review case records to ensure that services are provided according to the assessed care plan and DFTA standards 
  • Lead monthly group supervision and case conferences on weekly basic 
  • On key client cases, maintain documentation of provided supervision sessions 
  • Assist Social Work staff and nurses with referral processes, crisis intervention, coordination of care, and connections to community providers 
  • Directs case managers in developing corrective action plans in ongoing case management files, incident reports and in-home visit reports 
  • Conduct new case assignments for social services staff caseload, including reassignments of terminated/resigned supervisees 
  • Supervise staff and volunteers on telephone reassurance, escort, friendly visiting and coordinate recordkeeping of services and document their activity if necessary 
  • Supervise and train MSW interns if it would benefit the agency/program/clients 
  • Provide training to new staff members if necessary 
  • Substitute Senior Center Director in Director’s absence 

 

4. Remote Duties: 

  • If necessary, provide all case assistance duties remotely 
  • Provide remote supervision for social workers, program aides, administrative assistants, student interns and case assistants 
  • Meet with clients as needed at senior center for document drop off /pick up of documents for review and assistance from homebound clients if they are unable to complete drop off at the center

Qualifications/Educational Requirements 

  • LMSW, or LCSW required 
  • SIFI Certified, or SIFI-eligible 
  • Fluent in second language (especially Spanish or Cantonese/Mandarin) preferred 

 

Skills, Knowledge and Abilities 

  • Sensitivity to the strengths and needs of older adults, compassion and ability to communicate with clients who may have disabilities and/or mental illness 
  • Experience in supervision and leadership of professional staff, graduate school interns and volunteers strongly preferred
  • 3 plus years of experience working in a professional environment with older adults, community setting, healthcare, geriatrics or mental health 
  • Able to successfully manage multiple projects and tasks. 
  • Strong verbal and written communication skills 
  • Able to write and audit reports in clear and concise manner 
  • Effective leadership and coaching skills 
  • Demonstrates resourcefulness, diplomacy, and analytic skills 
  • Maintain cooperative working relationships with social services team 

 

Computer Skills: 

  • Strong computer skills, proficient with Microsoft Office Suit programs 
  • Able to learn and use new software 
  • Knowledge of DFTA online Data Base Peerplace ( STARS) preferred 

 

Physical Requirements 

  • Ability to greet clients in hallway to accompany to office 
  • Ability to travel to homebound clients and hospitals if needed 

 

Work Environment 

  • Senior center environment 
  • Shared office space 
  • Second floor accessible by elevator 
  • Occasional home visits and fieldwork to accompany participants to appointments or to follow up post-hospitalization 
  • Remote work if necessary
