Organization Overview: The Stanley M. Isaacs Neighborhood Center (Isaacs Center) is a non-profit, multi-service organization focusing primarily on the needs of children and low-income families, out-of-school and out-of-work youth, and aging New Yorkers. We operate at community centers located in the neighborhoods of East Harlem and Yorkville and seek to deliver programs that are impactful, innovative, and intergenerational.

 

Position Summary: The Family Development Coach will provide case management services to older adults living in the program catchment area and surrounding community. This will include developing and implementing family development plans with emphasis on the overall wellbeing. The ideal candidate will also conduct outreach to promote the program and identify new participants. Provision of services will be in accordance with Isaacs Center policies, contractual guidelines, and standards.

 

Schedule: 35 hours per week

 

Goddard Riverside and Isaacs Center follow the CDC and NYS recommendations to prevent the spread of COVID-19, and are now requiring all new hires to be fully vaccinated against COVID-19 unless they have a qualified exemption.

 

Roles, Responsibilities, and Essential Duties

Primary Duties:

  • Provide case management services to older adults residing within the program catchment area and
  • surrounding community which include: applying for government benefits and entitlements, advocating for
  • the clients’ medical, mental health, housing-related and other needs, and coordination of care
  • Develop and implement yearly family development plans (FDP) with the goal of improved outcomes and
  • quality of life
  • Assist with outreach efforts to increase participation

 

Qualifications: The successful candidate for the role of Family Development Coach should have

  • Demonstrated experience providing exemplary case management, facilitating group work, and managing data.
  • Program outreach knowledge is key.
  • Excellent interpersonal, communication, time management, organizational, and teaming skills are essential.

 

Education/Skills/Experience:

  • Bachelor’s Degree required; MSW or LMSW a plus
  • Fluency in Spanish is required
  • Must be comfortable using Microsoft Office products and have the ability to learn multiple databases for documentation purposes
  • Prior case management experience strongly preferred
  • Previous experience working with older adults strongly preferred
  • Ability to handle confidential information with discretion
  • Ability to work non-traditional hours (e.g. evenings, weekends, holidays) when needed
