Greenwich House will launch the Workforce Training & Resource Center in Fall 2022, which is envisioned as a programming, support, and activity hub for partnerships that will bring a broader array of services to the West Side. The Center will focus on curating skills trainings and services for adults and older adults and build opportunities for meaningful intergenerational engagements.

Position Summary:

This Case Manager works across the Health Services Division; CRW clinic and Workforce Development and Resource Center, delivering case management services to a clientele ranging from 18 to 100+ years old to assist in accessing and navigating the systems that impact the social determinants of health such as employment, economic, education, health, housing and social supports. This is a full-time (35hrs) case management staff position that offers flexible scheduling Monday through Friday –6:30am to 7pm, and the ideal candidate should be able to work days and evenings. Services are delivered in person and via Telehealth. The case manager will empower clients to lead enriched lives as they cope with issues throughout the life cycle including but not limited to mental health, medical health, substance use recovery and the aging process.

Primary Responsibilities:

  • Provide direct case management services for application/renewal of benefits and entitlements. Refer clients to appropriate community resources, e.g. securing identification, employment, legal, housing, accessing healthcare services, life planning and assist with applications and documentation.
  • Participate in weekly supervision and interdisciplinary staff meetings.
  • Timely completion of documentation in the electronic health record and other required reports to comply with: DFTA; NYS-OMH & OCF, NYC-DOH and OASAS regulations and requirements to include but not limited to: incident reports, progress notes: and encounter forms.
  • Report to the administration about recipient’s unmet needs.
  • Serve on the agency’s committees as assigned.
  • Immediate notification of Directors and appropriate clinical staff of reportable incidents and at risk behaviors.
  • Participate in ongoing professional training for continuing education units and other professional development as mandated by the agency or recommended by supervisor.
  • Perform other tasks and special projects as assigned by the direct supervisor.
  • Collaborate with Peer Advocate, Clinicians and Home Health Aides on shared clients

Qualifications:

  • MSW or higher with 1-3 three years clinical social work experience.
  • Experience in mental health and/or adult mental health, trauma informed care and harm reduction approach to substance use recovery.
  • Fluent with benefits and entitlements
  • Must comply with the agency’s standard of ethics, conduct and organization policies.
  • Work collaboratively with interdisciplinary team to accomplish program goals and adapt to the changing program environment.
  • Must be able to establish priorities and a course of action for handling multiple tasks.
  • Fluent in English and Spanish

COVID-19 Vaccination Requirement:

In order to protect our staff and to provide safe and reliable services to our clients, students, members, program participants, and the communities that we serve, as of October 29, 2021, we are requiring all Greenwich House and Barrow Street Nursery School new hires to be fully vaccinated against COVID-19 prior to their start date. We will consider exceptions for religious and medical reasons, where appropriate. “Fully vaccinated” means you must have both doses of a 2-dose vaccine and two weeks have elapsed since the second dose, or have received 1 dose of a 1-dose vaccine and two weeks have elapsed since the dose. Proof of vaccination status must be submitted prior to your start date and includes CDC Vaccine Card, NYS Excelsior Pass, NYC Vaccination Record, and other official Vaccine Record.
