Job Path is seeking a Youth Eligibility Specialist to assist students with disabilities who are about to transition out of high school. The ideal candidate will be organized, personable, and have excellent follow through. 

REPORTS TO: Program Manager – Transition Services 

RESPONSIBILITIES:

  • Work closely with Job Path’s Program Manager of Transition Services and Transition Coordinators in NYC public schools to identify graduating students with an interest in Job Path’s employment services.
  • Assist individuals in obtaining ACCES-VR eligibility (NY State Vocational Rehab agency); this will include but is not limited to assisting with submitting applications, communication with staff at ACCES-VR, setting up appointments, meeting with participants and their families, and collecting documents from families and Transition Coordinators.  
  • Work with Job Path’s Intake Coordinator, School Transition Coordinators and the Program Manager of Transition Services to assist people in all aspects of obtaining OPWDD eligibility, including but not limited to:
  • schedule and accompany students to appointments required for service eligibility when necessary 
  • collect documents and maintain spreadsheets detailing out where participants are at in eligibility process; provide ongoing and frequent update
  • in tandem with Job Path’s Intake Coordinator, maintain ongoing communication with Care Management agencies

In addition: 

  • Providing 1:1 support and co-facilitate groups as we engage in a Discovery process with students, a process in which we get to know individuals needs, interests and strengths to determine their vocational goals 
  • Submission of case notes, ACCES reports, and adding information to Vocational Profile 
  • Assist in leading workshops for students and parents on issues related to school to adult transition services 
  • Make site visits to schools that partner with Job Path
  • Continue to develop and grow ongoing the partnership with District 75

Location: At the present time, this is a hybrid remote position, requiring some presence in Job Path’s midtown Manhattan office. This position requires travel around the city to provide occasional support and to do community development work. Once the public health emergency is over, the job will be office-based with some flexibility.

HOURS: 17.5 hours/week 

Qualifications: High school diploma is required. Excellent written and verbal communication skills, experience with administrative work and record keeping, basic knowledge of Google platform. A background in working with people with disabilities and advocacy preferred.

Salary: $24/hr approved for 17.5-20 hours per week. 

For internal applicants only, adjusted salary to reflect half time at current position and half time at Project Planner position ( +$23,000). If combined with Direct Support Professional position, annual salary will be established at $38,600. This position is non-exempt.
