Title:               Licensed Practical Nurse  

Location:      820 Residential Stabilization & Rehabilitation

Salary:        $55,000 – $60,000

Schedule       Three 12/hr Shifts per week. 7pm – 7am. Rotating Weekends

 

Program Description:

 

The Program is an OASAS part 820 residential stabilization and rehabilitation program. Consumers receive substance use treatment in accordance with 820 rehabilitation residential services. They also receive services from nurses, psychiatrist and medical doctor. The program is designed as a holistic and highly individualized model of treatment and focuses on client-centered and trauma-informed approaches to helping people with substance use and co-occurring mental health disorders.

 

Essential Duties and Responsibilities:

The essential duties of the LPN include but are not limited to the following activities:

 

  • Under the supervision of the SUD Nurse Manager and SUD Assistant Director of Nursing, process admission screenings, discharges, transfers, and perform health care coordination for residential program services to the clients as per OASAS Part 820 Residential Stabilization & Rehabilitation.
  • Manage client care: including but not limited to daily vital signs, ppd placement, medication administration, referrals for rapid testing, and follow ups of medical and psychiatric appointments.
  • Provide oversight and monitoring of controlled substances and medication to promote organized and efficient medication tracking, refills, and disposal of medication, as needed.
  • Assist with managing the symptoms of withdrawal for clients who are in the stabilization element of care per medical providers orders.
  • Responds to Emergencies followed up by Incident Reports. 
  • Document all nurse intakes, follow ups, and discharges in electronic health record system.
  • Maintains accurate and timely telephone and written correspondence.
  • Coordinate with medical providers and psychiatrists to inform decisions regarding management and treatment of problems identified and provides care & transcribes orders by medical providers.
  • Participates in weekly case conferences and transdisciplinary meetings regarding client updates and modification of treatment as necessary according to health and wellness concerns.
  • Understands and can effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
  • Ensures and/or remains in compliance with local, state, and federal regulation, i.e., OASAS, NYS DOH.
  • Provides ongoing health education and groups to support clients.
  • May be required to participate in staff meeting or professional development activities as needed.
  • Completes other duties as assigned.

 

Qualifications:

  • New York State Licensed Practical Nurse
  • Current certification in CPR/BLS
  • Working with MICA and/or homeless population and experience in addiction medicine, ambulatory or hospital-based withdrawal service preferred.
  • Must have excellent oral, writing and listening skills.
  • The ability to work independently, prioritize tasks, and create and coordinate schedules. 
  • Must have the ability to interface with clients as well as all levels of staff.
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
  • Must have experience with electronic health records and knowledge of Microsoft Office
