Senior Case Manager – Streetwork LES
Summary:
Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach. Under this umbrella, Senior Case managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). Services provided by Senior Case manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case manager also works alongside program leadership in providing input towards program planning to meet the needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short and long term care necessary to meet the wide range of needs related to homeless adolescents.
Essential Job Functions:
- Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients.
- Provide ongoing safety assessment and risk management, crisis intervention, crisis counseling, emotional support and case management services of complex client cases, utilizing a trauma-based intervention (ARC).
- Conduct PTSD and substance use screening utilizing screening tools to determine clients’ eligibility for specialized mental health and substance use support services.
- Advocate on behalf of clients with both internal and external partners to increase clients’ access to benefits and resources; provide accompaniment as necessary and requested by clients; act as authorize representative on client’s case as necessary.
- Enroll clients for Public Assistance & SNAP/Food-Stamps by completing on-line application as needed.
- Provide initial assessment to youth in determining eligibility for SSI by completing Co-Occurring SSI screening tool and SOAR.
- Assist in determination of client’s eligibility for government-financed housing programs and benefit programs; complete electronic application for clients pursuing supportive housing and other government subsidized housing.
- Complete ECAPs & other Emergency Grant applications to help clients acquire first months’ rent and/or security deposit, rental arrears assistance, food, work uniform and transportation.
- Assess and recruit clients for assistance with job readiness training; make referrals and linkages for the client to job readiness and educational resources as appropriate.
- Facilitate weekly-based curriculum job readiness groups, sexual health, HIV/AIDS awareness, Independent Living Skills and Trauma based Interventions.
- Conduct bi-monthly presentations on Job Readiness in drop-in center, at community outreach programs and to local businesses.
- Work in collaboration with therapist and SSW to help develop treatment plans for youth with special needs experiencing severe mental health challenges.
- Provide assistance to SSW in facilitation of clinical group process and debriefs
- Collaborate with SSW in identifying trainings around staff development
- Provide support to Supervising Coordinators and Coordinators in absence of directors around scheduling and oversight of the space.
- Maintain accurate and timely documentation of case files, including monthly and quarterly funder and partner reports.
- Participate in staff meetings, case conferences, supervision meetings, trainings and other meetings as required.
Secondary Job Functions:
- Manage events in the Drop-In Center, monitor entrance to the program, oversee concrete support to clients, provide transportation to NYC Runaway and Homeless Youth Programs and/or oversee meal preparation.
Interacts with:
- All Streetwork program sites and staff
- All internal Safe Horizon programs and departments that can comprehensively support the client and case plan
- All external agencies and organizations that can provide appropriate services to the client
Knowledge and Skills:
Required:
- Compassionate approach to others and openness to new ideas
- Dynamic energy, able to multi-task and negotiate multiple demands on attention
- Non-judgmental approach and ability to empathize with others
- Clear and concise verbal and written communication skill, excellent interpersonal and organizational skills
- Genuine interest in working with disenfranchised youth and young adults
- Basic understanding of computer and systems for managing case records
- Understanding of social service systems and how to provide advocacy for clients navigating systems
- Basic knowledge of Supportive Housing and SSI eligibility process
- Ability to complete psychosocial reports and conduct assessments using psychiatric reports
- Demonstrate the ability to work independently
Education:
Required:
- Bachelor’s Degree , creative arts, liberal studies or any field related to working with youth +2 years experience working with youth or High school diploma or equivalent + minimum of 6 years experience working in social service settings with target population.
Experience:
Required:
- Experience working in a social service setting, OR
- Personal experience with street survival activities (homelessness, gangs, drug economy, sex industry) with three years separation from involvement in these activities. Deep understanding of the relationship between personal experience and professional standards.
- Experience working with youth who have Co-occurring Disorders
Desirable:
- Bi-lingual Spanish
- Understanding of harm reduction perspective
- At least 1 year working with the homeless population, youth, or harm reduction
- Experience in helping to develop and implement programming
- Experience in developing Evidence Based Interventions in trauma informed care
