The Doe Fund is a leading New York City non-profit organization that operates the enormously visible Ready, Willing & Able program, whose participants clean over 170 miles of streets and sidewalks every day. With a $69 million budget, 400+ staff members, and 46,000 individual donors, The Doe Fund is nationally acclaimed as a work-based model for solving homelessness and criminal recidivism. We empower people to break the cycles of homelessness, welfare dependency, substance abuse, and incarceration through innovative work and housing programs.
We are seeking a Senior Case Manager for The Doe Fund’s newest permanent supportive housing for homeless individuals and families living with HIV/AIDS. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents’ independence while ensuring their physical and emotional security and well-being. This 255-unit residence will provide housing for 153 individuals and families referred by the New York City HIV/AIDS Services Administration (HASA) and the rest 102 units will be apartments for moderate-income households.
The Senior Case Manager is responsible for providing supportive case management services and counseling to the most vulnerable residents referred by HASA as well as oversight and facilitation of the therapeutic groups. The successful candidate will have strong clinical skills and knowledge of the substance abuse and mental health treatment modalities along with an experience working with the HIV/AIDS population.
RESPONSIBILITIES:
- Maintain a caseload of the most vulnerable individuals who require specialized services and provide supportive case management services to them.
- Oversee and provide assistance with the development and facilitation of therapeutic groups for residents
- Maintain resident records in compliance with agency and funding standards
- Conduct and review assessments
- Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency
- Monitor and ensure residents are linked to healthcare providers
- Provide counseling and crisis intervention as necessary
- Assist residents in securing and maintaining benefits
- Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
- Monitor and assist residents with rent payments and budgeting
- Conduct regular unit inspections
- Educate residents about approaches to mental health and substance use
- Compile resources on mental health information and treatment strategies
- Attend ongoing training to enhance the services provided to the residents
- Assist with completion of reports as needed
- Other duties as assigned
QUALIFICATIONS:
- A Master’s Degree in Social Work or related field from an accredited college or university. LMSW or LMHC preferred.
- Minimum of five years of post-Masters experience
- The ideal candidate must have a working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness
- Excellent clinical and case management skills including knowledge of benefits in the NYC Metro area
- Sensitivity to community issues and needs
- Outstanding interpersonal and communication skills, both oral and written
- Strong ethical standards, confidentiality and personal integrity
- Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)
