Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and criminal recidivism by providing holistic services, housing and work opportunities to the under-served populations including the formerly homeless and incarcerated, disconnected youth, veterans, and people living with AIDS. Our flagship transitional work program– Ready, Willing & Able– integrates paid work, occupational training, and holistic supportive services to facilitate an individual’s return to mainstream society.
The Doe Fund’s Scatter Site Return Housing Program (SSR) focuses on assisting chronically homeless individuals in their transition from the shelters to the community living. The services include relapse prevention and crisis intervention using the principles of Harm Reduction and Motivational Interviewing, education on self-advocacy, community integration, safety, money management, socialization, employment skills, and referrals to appropriate treatment facilities. Through the Scatter Site supportive housing program, clients are moved into apartments located in the Bronx, Brooklyn and Manhattan areas.
The Senior Clinical Case Manager is responsible for providing supportive case management services including relapse prevention and recovery readiness to the Scatter Site Return clients, as well as consultation and supervision to the staff about substance abuse and mental health treatment modalities.
RESPONSIBILITIES:
- Maintain a reduced caseload of clients deemed in “high risk” category (active substance abuse and/or mental health issues); conduct home visits and provide supportive individual counseling to these individuals.
- Monitor and assist clients with rent and utility payments.
- Assist clients in accessing and maintaining benefits.
- Conduct apartment inspections and address any maintenance issues with property management staff to assure safety of the apartments.
- Maintain client records and complete a variety of charting documentation as scheduled.
- Develop and implement individual service plans and assist clients with achieving their goals geared toward self-sufficiency.
- Develop and facilitate support groups for clients to address substance use and mental health issues.
- Provide regular consultation and supervision (individual and/or group) to the staff about approaches to managing substance use and mental health symptoms as it relates to their clients.
- Initiate, monitor and follow-up on referrals to detoxification/drug rehabilitation and mental health facilities for those clients in need of such intervention.
- Maintain a liaison with community services including detoxification/drug rehabilitation, substance abuse treatment programs, and mental health programs.
- Participate in weekly individual supervision and staff meetings.
- Perform other duties as assigned by the supervisor.
QUALIFICATIONS:
- Master’s Degree in Social Work or related field with a minimum of two years of experience in substance abuse and mental health counseling.
- License in Mental Health Counseling (LMHC) or in Clinical Social Work (LMSW/LCSW) is preferred.
- The Senior Clinical Case Manager must be knowledgeable about addiction and mental health issues.
- Strong organizational, writing and communication skills.
- Fluency in Spanish is required.
