The Door is an unparalleled model for youth development, offering a comprehensive range of integrated services within two sites for up to 11,000 New York City youth each year. At The Door, youth can access health care and education, mental health counseling and crisis assistance, legal assistance, college preparation services, career development, housing supports, arts, sports and recreational activities, and nutritious meals – all for free and under one roof. Position Overview:  

The Licensed Practical Nurse (LPN) will be responsible for assisting the Associate Medical Director and health center providers with various clinical functions in the Adolescent Health Center. 

Responsibilities:  

  • Assist with clinical functions of the Adolescent Health Center, to ensure the delivery of high-quality care to patients.  
  • Assist with inventory control and ordering medical supplies, medications, devices, and equipment.  
  • Coordinate equipment maintenance and inspections.  
  • Coordinate the Vaccines for Children (VFC) program.   
  • Assisting providers with patient forms including work, school, sports physicals, and USCIS forms.  
  • Administer medication(s) and vaccinations based on written order(s) from licensed clinical providers.    
  • Facilitate the referral process including coordination of incoming and outgoing referrals, record retrieval, and appropriate follow-up.  
  • Assist with follow-up notification to patients of abnormal lab results, and reporting requirements to NYC DOHMH.    
  • Participates in patient-centered medical home care team collaboration, including team huddles. 
  • Assisting providers with rooming patients, patient intake, taking vitals, and discharge planning. 
  • Performs other duties as assigned.    

Qualifications:  

  • Graduate of an accredited Licensed Practical Nursing Program. 
  • Current, unrestricted NYS License as an LPN 
  • One year experience working in a community health center, or outpatient setting.    
  • Venipuncture skills a required 
  • Proficiency in working in a multi-disciplinary, fast-paced environment. 
  • Resourceful problem solver with the ability to manage multiple, concurrent projects. 
  • Exceptional oral and written communication skills. 
  • Strong PC skills, including skilled in Electronic Health Records, preferably eClinicalWorks, and proficiency with Microsoft Office.  
  • Bilingual English / Spanish or French strongly preferred 

Work Schedule: Full time, Monday – Friday, 10:00 a.m. – 6:00 p.m., 35 hours a week, some evening hours may be required

Salary: $28.00 – $34.00 per hour – This is an 1199 Union position, that comes with a generous benefits and PTO package
