Title:   Program Director                                  

Unit: Freedom House South Bronx                 

Report to: Senior Director of Transitional Housing

Salary Range: $80,000-85,000

Status: Full-Time; Regular; Non-Exempt

Location: 1171Washington Avenue, Bronx NY

Days/Hours: Daytime and Evening hours    

           

Position Summary:

Under the supervision of the Senior Director, the Program Director ensures that program services and quality care are provided daily.

 

Essential Duties and Responsibilities:

  • Oversee day-to-day operations of case managers, housing specialists, senior case manager, and residential aides. 
  • Maintain, and implement strategic operational systems that will enhance programmatic reports and evaluation.
  • Ensure quality and detailed documentation of case notes, service plans and referrals for care
  • Facilitate weekly participant community meetings, this requires the ability to work one evening per week.
  • Provide effective and professional coordination with internal and external providers related to participants care plans and services.
  • Review referrals for placement within transitional housing.  Collaborate with court advocates, and behavioral health staff.
  • Act as liaison internally within Fortune to ensure continuity of treatment
  • Research, evaluate, and maintain linkage agreements with outside client service resources;

 

 

Participate in and maintain growth oriented strategies that increase the program retention and efficacy,

  • Participate and encourage staff to attend Fortune advocacy events.
  • Develop extensive base of referral agencies that provide the following services: mental health, health, dental care, substance abuse treatment, education, employment, family services, alternatives to incarceration, housing and gay/lesbian/transgender counseling etc.
  • Coach high performing team members, empowering them to elevate their level of responsibility, span of control and performance,
  • Submit weekly and monthly reports to the Senior Director in a timely manner;
  • Support the Senior Director in the hiring, onboarding, training, and counseling of new employees;
  • This role requires 24hr on-call for emergencies and support.
  • Perform other duties as needed.

SUPERVISES:           Senior Case Manager, Program Coordinator, Housing Specialist, Case Managers

 

Qualifications

Qualifications:

  • Master’s Degree preferred or BA with 5 years equivalent work experience in a similar setting working with underserved populations.
  • Knowledge of case management resources for formerly incarcerated individuals and their collaterals
  • Strong computer skills with a good working knowledge of Excel and Microsoft word.
  • Excellent written and oral communication skills;
