Job Description

Under the direct supervision of the Clinical Supervisor, the Social Worker will provide a comprehensive range of social work services to the residents of W. 108 Street, enabling the residents to live safely and as independently as possible.   The Social Worker works as part of the social service team in tandem with the Clinical Coordinator and the Program Director, in developing 1:1 relationship with residents, provide counsel on mental health issues and physical health issues.

The West Side Federation for Senior and Supportive Housing, Inc. (WSFSSH) is a community-based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person.  WSFSSH develops, manages, and provides housing for low- income older persons, many of whom live with mental illness and/or have experienced homelessness.   WSFSSH is an Equal Opportunity Employer (EOE) 

Key Responsibilities

  • Assist residents in obtaining and maintaining appropriate medical and mental health treatment.
  • Assist residents in obtaining and maintaining entitlements.
  • Assist residents in obtaining and maintaining home care.
  • Assist residents in establishing and/or maintaining family and social contacts.
  • Through in-house and community referrals, assist residents in participating in socialization activities.
  • Assist residents with money management, budgeting, income maintenance etc. to insure on-time payment of rent.  
  • As a member of the Intake Committee participate in applicant interviews and make admission recommendations.
  • Follow up with resident and other service providers during out of facility stays such as hospitalizations and/or short-term rehabilitation placements.   
  • Maintain case records, including case notes, service, support or treatment plans, psychosocial histories, discharge summaries and plus updates within the time frames required by WSFSSH and the contract/regulatory agencies.  
  • Maintain the thoroughness and quality of chart documentation required by WSFSSH, the regulatory/contract agencies and good social work practice.
  • Oversee the discharge planning process for residents as applicable, including the completion of all necessary discharge forms and/or applications to other facilities.
  • Provide individual counseling as needed.
  • Provision of crisis intervention services as needed.
  • Provision of group work services for residents.
  • Participate in the development of recreational, social and educational activities for residents.  

 Other Duties  

  • Maintain high level of respect and care for residents
  • Work cooperatively with other staff members
  • Share information about resident progress, needs, and problems with other staff as appropriate, guided by the WSFSSH Confidentiality Policy.  
  • Develop and maintain competency in AWARDS, CARES and other software and data bases as required.
  • Attend training sessions and conferences as required for enhancement of job skills
  • Implement emergency procedures as necessary
  • Attend supervision meetings, team meetings and staff meetings.
  • Participate in the development of staff trainings
  • Other duties as assigned.

 Qualifications 

  • BSW Required. MSW/LMSW preferred. 
  • Fluency in written and oral English and Spanish preferred.
  • Experience with the elderly and people with mental illness preferred
